CONTRACT CANCELLATION FORM

On this date, the following clients release M Farmer Productions LLC dba f/8 Studios and Michael Farmer Photography and all personnel working for M Farmer Productions LLC, from all liability, responsibilities, and commitments regarding the event of the Clients on their event date.

As agreed upon in the initial wedding contract, the Client may cancel the contract by giving written notice to M Farmer Productions LLC. The previously paid 50% deposit is non-refundable per the initial wedding contract.
